  The following armamentarium is present:<\/strong><\/span><\/span><\/p>\n

___   Gloves, wrist watch with second hand, digital thermometer, disposable<\/span><\/p>\n

         plastic sheaths, disinfectant,  2×2 gauze sponges, stethoscope,<\/span><\/p>\n

         sphygmomanometer, patient\u2019s chart, pen<\/span><\/p>\n

Steps:<\/strong>   0 \u2013 10 points each<\/span><\/p>\n

Obtains & Records Pulse:<\/span><\/strong><\/span>          <\/span><\/span><\/p>\n

___  1.  Seats patient upright<\/span><\/p>\n

___  2.  Washes hands and gloves up<\/span><\/p>\n

___  3.  Explains procedure<\/span><\/p>\n

___  4.  Instructs patient to rest wrist on arm of chair or counter<\/span><\/p>\n

___  5.  Locates radial pulse with pads of first three fingers over wrist<\/span><\/p>\n

___  6.  Gently compresses radial artery so that pulse can be felt<\/span><\/p>\n

___  7.  Using second hand on watch, counts number of pulsations for 30<\/span><\/p>\n

              seconds, and notes any irregular rhythm patterns<\/span><\/p>\n

___  8.  Keeping fingers on pulse, counts rise and fall of chest for 30 seconds<\/span><\/p>\n

___  9.  Multiplies pulse and respiration counts by 2 and records on paper<\/span><\/p>\n

             along with any evidence of rhythm irregularities<\/span><\/p>\n

Obtains and Records Temperature:<\/span><\/strong><\/span><\/p>\n

___10. Places a disposable sheath over tip of digital thermometer<\/span><\/p>\n

___11. Explains procedure to patient<\/span><\/p>\n

___12. Turns on thermometer and when display shows it is ready, gently<\/span><\/p>\n

            places tip under patient\u2019s tongue<\/span><\/p>\n

___13. Leaves in place until display indicates final reading, then removes<\/span><\/p>\n

            from patient\u2019s mouth<\/span><\/p>\n

___14. Records the reading on paper<\/span><\/p>\n

___15. Turns off thermometer and disinfects thermometer as recommended<\/span><\/p>\n

             by manufacturer<\/span>      <\/span><\/span><\/p>\n

Obtains and Records Blood Pressure:<\/span><\/strong><\/span><\/p>\n

___16. Explains procedure to patient and instructs patient to extend arm at<\/span><\/p>\n

            heart level<\/span><\/p>\n

___17. Places blood pressure cuff around patient\u2019s arm about 1 inch above<\/span><\/p>\n

            antecubital space and secures it in place<\/span><\/p>\n

___18. Opens valve to expel any air from cuff<\/span><\/p>\n

___19. Positions ear pieces of stethoscope in a forward manner into the ears<\/span><\/p>\n

___20. Places diaphragm of stethoscope over brachial artery, securing in<\/span><\/p>\n

            place with thumb.  Places other fingers under elbow to hyperextend <\/span><\/p>\n

            artery<\/span><\/p>\n

___21. Locks bulb valve and inflates cuff using pumping pressure on bulb<\/span><\/p>\n

            until inflation level reaches a reading of 160 \u2013 180<\/span><\/p>\n

___22. Deflates cuff at a rate of 2 \u2013 4 millimeters of mercury per second by<\/span><\/p>\n

            slightly releasing the bulb valve<\/span><\/p>\n

___23. Notes reading when first detecting a distinct thumping sound<\/span><\/p>\n

            (systolic pressure)<\/span><\/p>\n

___24. Continues to slowly deflate pressure listening to pulsing sounds to note when<\/span><\/p>\n

            all sounds disappear (diastolic pressure).<\/span><\/p>\n

___25. Rapidly deflates cuff and removes from arm<\/span><\/p>\n

___26. Correctly records reading on paper, indicating which arm was used<\/span><\/p>\n

___27. Disinfects ear pieces and diaphragm of stethoscope according to manufacturer\u2019s<\/span><\/p>\n

            directions<\/span><\/p>\n

___28. Removes gloves and washes hands<\/span><\/p>\n

___29. Records vital signs on patient\u2019s chart<\/span><\/p>\n
