Module VIII is in a custom workbook format that will be mailed to your office along with a typodont. Please pay accordingly on the registration form when registering for Module VIII. Include the fee for the typodont. Upon completion and return of the undamaged typodont, up to $90 will be refunded to the card that paid for the typodont. The skill sheets and notarized summary checklist will be included in workbook 4 of the packet.
